For the steps/mm you need motor / drive steps/rev and mechanical ratio. e.g. if you have 8000 steps/rev drive and 10mm lead screw that will be 10mm/8000 =0,00125 mm
Regarding acceleration, there is no formula, it depends on particular machine, drive, requirements, etc. In case of doubts start with conservative values like 500mm/s^2 and Jerk 50000mm/s^3. This should work for most cases, and you can then try more aggressive values like accel = 2500mm/s^2 and jerk 150000mm/s^3.
